Meridian's radiology team was reading studies in strict submission order, regardless of clinical urgency, which meant time-critical findings — a possible stroke, an unexpected mass — sometimes waited behind a backlog of routine scans simply because of when they arrived. Radiologists had no systematic way to flag likely-urgent studies without slowing down an already-stretched reading list, and any solution that required new training, a new interface, or a change to how they opened a study was a non-starter for a department already running at capacity. Leadership also needed any new system to produce a clear, auditable rationale for every triage decision, since misclassifying an urgent study has real clinical consequences.

We built a triage scoring layer that runs ahead of the existing reading queue, evaluating each incoming imaging study for likely urgency using a model trained on de-identified historical reads and confirmed outcomes. The system re-orders the radiologist's worklist so higher-urgency studies surface first, but it never auto-diagnoses or hides a study — every read still happens through the radiologist's existing PACS viewer, untouched. We added a confidence-scored explanation panel so radiologists can see why a study was flagged, which became essential for both clinical trust and the hospital's own internal audit process. The system integrates entirely on existing infrastructure, requiring zero changes to how radiologists open, read, or sign off on a study.
